SENIOR PEER PROGRAM

CREATING MEANINGFUL CONNECTIONS
The Senior Peer Program strives to improve the well-being of seniors by providing increased social connections with their peers. Fostering relationships among seniors can help prevent significant mental health problems such as major depression and debilitating anxiety.

Melanie Jacobson

SENIOR PEER PROGRAM MANAGER

Melanie is a Prescott native who returned to her hometown in 2003 to be close to her aging parents, her three sisters and their children. In 2022 Melanie joined Polara Health to become the manager of its Senior Peer Program, a volunteer-based community outreach to isolated seniors for companionship and friendship.

ABOUT SENIOR PEER

Peer Volunteers help other seniors identify ways that they can increase their social connections and improve their sense of meaning and purpose in their lives

Support Groups help seniors to develop connections by supporting each other through shared experiences and challenges. All groups are facilitated by a trained volunteer.

Our programs bring older adults together with one-to-one visits. Peer Volunteers help other seniors identify ways that they can increase their social connections and improve the sense of meaning and purpose in their lives.

All Support Groups are free, confidential and 
​facilitated by volunteers in the Quad-City area.

A safe and confidential place to share experiences living with low vision and how to navigate life with confidence and acceptance.

For those caring for a vision-impaired loved one.  Learn strategies not only for the care of loved ones but also how to take care of oneself.

MINDFULNESS MEDITATION
​CLASS

Learn a secular practice  that helps to pay attention to current experiences and accept them without judgement.  Mindfulness is beneficial to our psychological and physical well-being.
