Adults Primary Care:
Preventive screenings, women’s and men’s health services, lifestyle counseling, and comprehensive chronic disease management, including conditions such as hypertension, diabetes, asthma, high cholesterol, and thyroid disorders.
Geriatrics (Senior / Elderly Care):
Comprehensive care for older adults including chronic disease management, fall‑risk assessment, cognitive and memory screening, medication review, and support for healthy aging and independence.
Medically assisted treatment program (MAT)
Provide a combination of FDA‑approved medications with counseling and behavioral therapies to effectively treat substance use. This approach helps reduce withdrawal symptoms and cravings, supporting safer recovery and improved long‑term outcomes.

Dr. Manal Soliman, MD, MBA
Dr. Manal Soliman serves as Chief Medical Officer at Polara Health, where she provides clinical leadership focused on high-quality, accessible primary care and integrated health services for the community. A board-certified Family and Lifestyle Medicine physician, Dr. Soliman brings decades of experience in patient care, medical education, and healthcare leadership.

Cristal Apolinar
Cristal Apolinar is a Family Nurse Practitioner at Polara Health who is passionate about providing compassionate, patient-centered primary care. She earned her Master’s degree from Grand Canyon University in February and has been practicing as a Family Nurse Practitioner since May 2025. Cristal values building meaningful connections with her patients and takes pride in understanding and meeting their individual healthcare needs.
