Community Health Needs Assessment

The 2010 passage of the Patient Protection and Affordable Care Act (PPACA) ushered in new reporting requirements for health care organizations. Per IRC Section 501(r), private, nonprofit hospitals must:

  • Conduct a community health needs assessment (CHNA) at least once every three years on a facility-by-facility basis.

  • Identify action plans and strategies to address community needs identified in the assessment and report needs not being addressed (with reasons why such needs are not being addressed).

  • Report CHNA results to the public.

This community health needs assessment, which describes both a process and a document, is intended to document Polara Health’s compliance with IRC Section 501(r). Health needs of the  community have been identified and prioritized so that Polara Health may adopt an implementation strategy to address specific needs of the community.

Stress is hard to define because it means different things to different people; however, it’s clear that most stress is a negative feeling rather than a positive feeling. A well-known tool used by health professionals to measure the amount of stress someone’s experiencing is the Holmes-Rahe Life Stress Inventory

MATForce  is a substance abuse coalition that supports prevention programs for youth and family. Polara Health is an active partner in this coalition, and has been since its inception.

National/Regional/Local Resources for Mental Health Disorders

Mental Health America
This organization is one of the best resources for learning about mental illness and substance use disorders. They have information about anxiety, bipolar, signs of depression, Post Traumatic Stress Disorder (PTSD), Psychosis and more. They also offer free screenings for individuals to determine if they are experiencing symptoms of a mental health condition.

The National Alliance for Mental Illness (NAMI)
NAMI is another amazing resource for education, advocacy, and support. NAMI is the nation’s largest grassroots mental health organization dedicated to building better lives for the millions of Americans affected by mental illness.

Launched in 2011, MakeTheConnection.net connects Veterans, their family members and friends, and other advocates with mental health information, local resources, and Veterans’ own inspiring stories of recovery. Visitors can learn about the signs of problems with drugs and alcohol and find sources of support. The site is free and accessible to everyone.
