Arizona Tax Credit Donation

Arizona offers a dollar-for-dollar tax credit on your Arizona state tax return by making a donation to a Qualified Charitable Organization (QCO) of any amount.  The maximum credit allowed is $841 for married filing joint filers and $421 for single, heads of household, and married filing separate filers. Polara Health is a Qualifying Charitable Organization - QCO Code #20367.  

When you donate to Polara Health, you may claim your tax credit on Form 321. Should you need more information, visit azdor.com or ask your tax preparer.

IRA Qualified Charitable Distribution

Do you have an IRA? Have you reached the age of 70 ½? You can make a tax-free contribution of up to $100,000 directly from your IRA to Polara Health. This will qualify as your required minimum distribution. 

Contact your IRA plan administrator to begin making a gift to Polara Health from your IRA.

Give a gift that lasts more than a lifetime.  Through a gift from your will, trust, or other estate-giving methods, you can ensure that Polara Health’s work with the most vulnerable will continue for generations. Legacy-giving options can include:

  • Providing a bequest for Polara Health in your will or trust

  • Listing Polara Health as a beneficiary for your IRA or life insurance

Your appreciated securities or other real property can benefit those on their journey to wellness with Polara Health. You may donate stocks, bonds, real estate, or other assets.

Memorial or Honor Giving

Remember a loved one through a gift.  Continue the legacy of a person who has passed away through memorial giving in that person’s name.  You can ask to have a recipient be notified of your gift.
